The physical form has changed to a Pre-Participation Form that includes the physical form that athletes have to have filled out by a doctor. Both the English and Spanish version of those forms are linked below.

The Gfeller-Waller Concussion Awareness Act was drafted and implemented to protect the safety of student-athletes in North Carolina and was signed into law on June 16, 2011 by Governor Beverly Purdue.

There are three major areas of focus in the law and these include: education, emergency action and postconcussion protocol implementation, and clearance/return to play or practice following concussion.

If a student-athlete needs to complete this form, he or she will be informed of this by a coach.

This form is completed by parents/guardians to give permission for a student to participate in high school athletics. It must be completed every year.  A checklist for eligibility can be found here. These requirements are enforced by the North Carolina High School Athletic Association.

This is a form required for Catawba County Schools.

            CCS Athletic Participation Form

This pledge must be signed by the student-athlete and his/her parents.

               Student-Athlete Pledge Form

This is a new form required for the 2020-2021 school year. Athletes should turn this form in to their coaches.
